T4 physicians and administrators systematically falsified official records to conceal those murders. According to T4's own internal calculation, 70,273 disabled and psychiatric patients were killed at the six centralized gas-killing facilities between January 1940 and August 1941.

The false natural-death story was implemented as a bureaucratic system rather than an occasional lie to individual families. Each T4 killing center had a dedicated registry office. Staff manipulated dates to avoid conspicuous local clusters of deaths, transferred victim files between killing centers so that false places of death could be recorded, and sent relatives standardized notices and civil death certificates containing invented causes, dates and places. Urns requested by families did not contain the named victim's ashes. The machinery helped conceal the killing program and created false official records for tens of thousands of murders.
How the consequences followed
The deception did not consist of one identical false diagnosis. T4 doctors selected plausible natural causes for individual victims, while administrative staff also falsified dates and sometimes places of death. The stable institutional proposition was that the transferred patients had died naturally rather than being murdered.
This concealment was not completely successful. The killings became widely suspected, families challenged implausible notices and public and private protests emerged. The existence of growing public knowledge does not change the fact that the T4 apparatus continued to generate false civil records and official family communications.
